Job Description:
Job Description Job Description This position is responsible for
supporting individuals in behavioral health services by using
therapeutic techniques and advocacy to empower clients, promote
recovery, and connect them with resources by performing the
following duties. Essential Duties Conduct outreach in the
community; locate individuals identified as very high risk in order
to engage them and enroll in services, respond on-site to gun
violence hospitalizations and provide support to victims and
families Establish and maintain a positive and trusting
relationship with each client and use influence to help clients
make better decisions including desisting from violence Case
management: develop Life Plans with each client and document all
case management efforts, update individual case files, and make all
required entries into the data management system Job Duties
Provides support to individuals in need of behavioral health
services. Conducts assessments and develops individualized
treatment plans. Implements evidence-based interventions and
therapeutic techniques. Monitors progress and makes necessary
adjustments to treatment plans. Collaborates with a
multidisciplinary team to ensure comprehensive care for our
clients. Provides crisis intervention and support during
emergencies or high-stress situations. Advocates for clients and
connects them with appropriate community resources. Maintains
accurate and up-to-date documentation of client interactions and
progress. Facilitates group sessions and peer support meetings to
foster a sense of community and shared experiences among clients.
Educates clients on self-care practices and coping strategies to
empower them to manage their mental health and well-being. Provides
mentorship and guidance to new peers joining the program, helping
them navigate their roles and responsibilities. Acts as a liaison
between clients and healthcare providers, ensuring effective
communication and continuity of care. Maintains confidentiality and
upholds ethical standards in all client interactions, ensuring a
safe and trusting environment. Performs other duties as assigned.
Experience/Education A high school diploma or GED is required for
this role. In addition to the education requirement, this position
requires a minimum of two years of related work or lived
experience. An equivalent combination of education, training, and
experience may be used to satisfy these requirements. Travel
Requirement This position requires regular (21-50% of the position)
travel related to the job duties above. Travel will primarily
consist of day trips. Qualifications Ability to perform essential
