Overview: The positions of Care Manager (MSW and LCSW) focus on

Manager MSW The Clinical Social Worker collaborates with patients,
families, and the healthcare team to advocate for and address the
social and emotional needs of patients. Responsibilities include a
comprehensive range of social work services such as psychosocial
assessments, treatment planning, therapeutic interventions,
discharge planning, crisis management, and resource referrals. The
Clinical Social Worker works alongside the patient and their
support network, as well as interdisciplinary teams, both within
and outside the organization, to enhance patient care through
effective resource management. This role also involves leadership
in achieving patient goals and desired clinical, financial, and
resource outcomes, guided by the NASW Code of Ethics and the
organization’s mission and vision. Required Qualifications:
Master's Degree in Social Work from an accredited institution. Upon
hire: Washington Clinical Independent Social Worker Associate
License. Minimum of 1 year of experience in acute medical or mental
healthcare, either inpatient or outpatient. Preferred
Qualifications: ACM/CCM certification. Care Manager MSW Salary
Range: Edmonds (Edmonds, WA) Min: $45.66 Max: $69.47 Care Manager
LCSW Required Qualifications: Master's Degree in Social Work from
an accredited institution. Upon hire: Washington Clinical
Independent Social Worker License. Minimum of 1 year of experience
in acute medical or mental healthcare, either inpatient or
outpatient. Care Manager LCSW Salary Range: Edmonds (Edmonds, WA)
