Job Description:
Legacy Health is seeking a dedicated Nurse Practitioner for our
outpatient clinics across Oregon and Washington. This position
offers the opportunity to work in a dynamic, patient-focused

development. Perform comprehensive patient assessments, diagnose
health conditions, devise and manage treatment plans. Collaborate
with other healthcare professionals to coordinate patient care.
Educate patients and their families on preventive care, medical
issues, and use of prescribed medical treatments and/or
medications. Monitor patient progress and adjust treatments as
necessary. Participate in regular team meetings and contribute to
clinic initiatives and quality improvement measures. Maintain
patient records and adhere to health and safety policies. Provide
mentorship to new staff and participate in community health
initiatives. Strong clinical skills and proficiency in diagnosis
and treatment planning. Excellent communication and interpersonal
skills. Ability to work both independently and as a part of a
multidisciplinary team. Strong organizational skills and ability to
handle a significant patient load. Medical Insurance Dental
Insurance Vision Insurance Life Insurance 401k Current,
unrestricted Nurse Practitioner license in the state of Oregon or
Washington. ANCC or AANP board certification. BLS (Basic Life
Support) Certification required.
