Job Description Job Description Nurse Practitioner / Sleep
Medicine This position works in collaboration with physicians to
provide complete physical examinations, diagnosis, and treatment of
chronic sleep disorders. Nurse Practitioner will have a panel which
includes patients initially evaluated by a Sleep physician and
those which the Nurse Practitioner has independently performed the
initial evaluation of the sleep disorder. Our team works
collaboratively in a supportive environment where the physician is
always an available resource. Responsibilities : Assesses the
physical condition of patients by performing physical examinations
including ordering, interpreting and evaluating diagnostic tests
and obtaining medical histories. Formulates the medical plan and
prognosis based on patient's condition. Consults with physician(s)
regarding patient health care plans and treatment. Establishes and
documents the health care plan and prognosis, maintaining records
of each case and evaluates the effectiveness of the plan.
Determines when the patient has recovered from the disorder and
releases the patient. : Education: As required by licensure.
Experience: Clinical experience requirements with Sleep Medicine.
This is a part time position - Wednesday/ Friday - Contract rate
per RVU visits
