RN Coordinator - Infusion at Providence Health and Services
summary: The RN Coordinator - Infusion manages the planning and
coordination of infusion therapy referrals and provides phone
triage for patients in home, clinic, or alternate living settings.
The role involves collaboration with healthcare providers,
patients, caregivers, physicians, and payors to ensure effective
case management and direct nursing services. The position requires
Oregon RN licensure, home infusion nursing experience, and focuses
on delivering patient-centered care within Providence Home Services
in Oregon and Southwest Washington. Description RN Coordinator-
Providence Home Services- Oregon Schedule: Full Time/Days RN
Coordinator role consists of the planning and coordination of new
referrals. The position will also provide phone triage of existing
patients. The majority of the responsibilities will be carried out
onsite in office. May provide direct nursing services and case
management related to infusion therapy for patients in the home,
clinic, or alternate living site. This is accomplished through
collaboration and coordination with various providers, the patient
and/or caregiver, the physician and the payor. These services are
provided in the states of Oregon and/or southwest Washington.

Upon hire: Oregon Registered Nurse License Within 30 days of hire:
National Provider BLS - American Heart Association. Upon hire: Must
be licensed in states wherever care is delivered. 3 years Home
Infusion nursing experience. Preferred Qualifications: Bachelor's
