RN Coordinator - Infusion at Providence Health and Services summary: The RN Coordinator - Infusion at Providence Home Services in Oregon manages new patient referrals and provides phone triage for existing patients, primarily onsite. This role involves direct nursing and case management for infusion therapy in home, clinic, or alternate living settings, collaborating closely with healthcare providers, patients, and payors. The position requires licensure in Oregon or Washington and experience in home infusion nursing, delivering patient-focused care within a broad multi-state healthcare system. Description RN Coordinator- Providence Home Services- Oregon Schedule: Full Time/Days RN Coordinator role consists of the planning and coordination of new referrals. The position will also provide phone triage of existing patients. The majority of the responsibilities will be carried out onsite in office. May provide direct nursing services and case management related to infusion therapy for patients in the home, clinic, or alternate living site. This is accomplished through collaboration and coordination with various providers, the patient and/or caregiver, the physician and the payor. Required Qualifications: Upon hire: Oregon Registered Nurse License or Upon hire: Washington Registered Nurse License. Within 30 days of hire: National Provider BLS - American Heart Association. Upon hire: Must be licensed in states wherever care is delivered. 3 years Home Infusion nursing experience. Preferred Qualifications: Bachelor's Degree in Nursing. Why Join Providence?
