$3,000 hiring bonus for eligible external hires that meet required qualifications and conditions for payment . Schedule: Full time, Days, Monday- Friday (8:00am - 4:30pm) Responsible for East Portland area Manages and provides skilled physical therapy to patients/clients with a wide variety of diagnoses and disabilities in a home setting in accordance with a physician plan of treatment, taking into account individual patient special or age-related needs. Requires skills in patient/client evaluation and treatment, as well as professional consultations to achieve maximum patient/client independence in accordance with the individuals functional level and potential. Required Qualifications: Bachelors Degree from an accredited Physical Therapy Program. or Masters Degree from an accredited Physical Therapy Program. or D.P.T. from an accredited Physical Therapy Program. Within 30 days of hire: National Provider BLS - American Heart Association Upon hire: Oregon Physical Therapist License Upon hire: Washington Physical Therapist License - if practicing within this state 1 year Clinical experience in Home Health or healthcare setting, or active enrollment in the Providence postgraduate preceptor program. Pay Range: $ 48.52 - $ 67.27 The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
