*Cedar Sinai Park is looking for full-time and part-time Licensed Practical Nurses at our Skilled Nursing and Long Term Care community!**We are located at 6125 SW Boundary St. Portland, Oregon 97221, in the Raleigh Hills area of southwest Portland. Observes, documents and reports any significant changes in the resident's condition that warrants medical and/or nursing intervention. Assures the provision of nursing services and takes action to ensure fulfilling resident wishes and promoting the least restrictive alternative of care. Revises care plans daily as needed, by identifying problems, formulating measurable goals, and incorporating resident/family wishes and teaching; while respecting and promoting the resident to self-direct their own care. Administers medications to assigned residents as ordered by the physician, including intravenous services.*Required Education, Experience, Licenses, and Certifications* Graduate of an accredited School of Practical Nursing required.
